Athletic Clearance

All physicals must be dated on or after May 1, 2025
Students who participate in the following activities must complete the clearance process:
Sports, Dance, Cheer, Band, NJROTC and Performance Choir 
Must complete the athletic clearance process before participating in physical activity.

Please complete ALL 3 steps:

1. Physical Evaluation Form (linked at the bottom of the page)

• Medical Eligibility Form
• MUST be completed by a licensed MD, PA, DO or Surgeon
• MUST have physician signature AND office stamp
• Kaiser physical forms are accepted

2. Go to Home Campus
   Complete ALL 5 sections

• Medical Insurance coverage is REQUIRED
• If needed, information is available in the Front Office or go to Myers Stevens Enrollment
• For those students without an ID number, please enter  0000
• Choose “La Habra” from the list and “2025-2016” as the school year
• Once completed, please print the “Confirmation Page” and sign

3. Scan the Medical Eligibility Form (1 Page) and the SIGNED Confirmation Page to the website. (Please see below)
